How do I share my test results and documents with my doctor?

Your patient portal associated with your email is the only way to share your documents with your doctor. It is a secure system that complies with the privacy laws in Ontario.

The link to log into your Patient Portal and upload documents is: https://portal.outsmartemr.com/ . Use the email and password you used when you created the account. If you do not recall that information, choose "forgot password" and it will allow you to reset it.  

Under the documents and uploads section, you can drag and drop your PDF or other documents into your patient chart.

We cannot access your lab work by using your username and password for you lab account or your hospital access. We also cannot view lab work that are sent as links to your lab account. You need to download the PDF of your lab work and upload it into your patient file for your doctor to be able to view it.
