Where do I find my treatment plan?

You can log into your Patient Portal to view your treatment plan (and labs or other documents if applicable) from your visit. You will not require a new access code, but here are the instructions for logging in:

1. Go to https://portal.outsmartemr.com, and log into your account.
2. If you have forgotten your password click on "forgot password" to reset. If you know your password, enter and click login.
3. That's it! Your dashboard will refresh, and you are now connected to your medical data!
4. Your Treatment Plan is contained in the Documents tab.
