How do I access my patient portal?

After you book your intake appointments, you'll receive an email from our patient software with the subject line “Patient Portal Access Link”.  This will prompt you to register for a new account. This action is time-sensitive.


Simply click “Register Here”, fill out all the information and create your own password.

In the Patient Portal, you’ll see a SmartForm section where you will find two forms.


Start with the Policies & Consent Agreement. It's the shorter of the two forms, and must be done as your Doctor cannot begin your appointment without it. It also explains our 2 business day cancellation policy. We do require 2 business days' notice to cancel or reschedule an appointment and if you were to call with less time than that you will be responsible for the full fee of that missed visit. It is a policy we’re firm on so I like to point it out verbally just so that everyone is aware. 


The second form is your Initial Intake Form which is a bit more lengthy but you can save your progress as you go through it. Typically it takes patients about 20 to 30 minutes to complete it.
