How can I get a copy of my chart for an other doctor?
We cannot release records to you or another provider without your consent. If another provider would like access to your full chart, you can sign a release with that provider for us to release your chart to them. Charts can be released to patients for a fee of $50. This accounts for the time (approximately 30-60 minutes) it takes to manage the paperwork, copying and secure sending of your chart.
Once invoices for chart-release are paid, the chart will be released to your new provider in 7-10 days.
For many instances, your patient portal containing your treatment plans and lab tests are sufficient for a new provider to use. You may not require your entire medical chart to be forwarded. Discuss what is necessary with your new provider and this may help you avoid administrative costs associated with chart copying. Our goal in using our patient portal is to give you un-inhibited access to your medical records. The additional notes your ND makes in your appointments and your intake forms may not be needed by your other providers and take significant administrative time to copy and share.
